 Sacramento City and County, Community Service Planning Council have a Ten Year Plan to eventually eliminate chronic homelessness by providing permanent housing and coordinate services to help those participating in the program to achieve maximum self-sufficiency.This Ten Year Plan was established in 2007 with a Policy Board with public and private sector community leaders who are responsible for the planned direction, oversight and support for the Ten Year Plan and homeless services. In addition, an Interagency Council consisting of government agencies, service providers and community stockholders who have the responsibilities for planning and coordinating service delivery. Interagency Council also includes ten committees focusing on improvements in the systems that affect homeless people of Sacramento County like the healthcare sector and criminal justice system. The definition of chronic homeless are those with disabling conditions, serious mental illness, developmental disability, chronic physical illness and substance use disorder and having been on the street or shelters for a year or more and also four or more episodes of homelessness. In the past, there was a 5 Year Plan to reduce the homeless in Sacramento County a plan that unfortunately was dissolved due to the lack of dedication by those involved. No follow up reports were done to tract the outcome of this plan and the plan was dissolved in 2007. A new Board and a Ten Year Plan was developed not only reduce homelessness in Sacramento but to address the problems and adopt a plan to eliminate chronic homelessness. The plan is to provide permanent housing, improved services and making these services and support available to those who are in the program. The success of the Ten Year Plan depends on the dedication and focus of those involved. Keeping in mind their roles of responsibilities for the continual planning, direction, and support until the completion of the vision of the Ten Year Plan.
